Our Maternity Stream Launch — Well-being Event with Professionals is coming up. You can download, print and bring these forms with you, or ask us for a paper copy.
For everyone taking part in the Maternity Stream Launch — Well-being Event with Professionals. Adults complete the participant section, and a parent or legal guardian completes the consent section for each child under 18.
Lets us know whether we may use photos or videos from the event, and for which purposes. Consent is optional and you can leave any box unticked.
For anyone helping out at the event or with the group. It explains what to expect, how we keep everyone safe, and how the volunteering arrangement works.
